MyChart Proxy Access

What is proxy access?

Proxy access allows you to securely communicate with the patient's physician's office on behalf of a family member or person under your care. This communication is done using a MyChart Account and is restricted to non-urgent matters. Proxy access is granted once an authorization form has been completed and processed by site where the patient is receiving care. Proxy access can be terminated by YNHHS/YMG at anytime.

Pediatric Proxy

Pediatric proxy access for MyChart allows a parent or guardian to securely communicate on behalf of an YNHHS/YMG pediatric patient who is under the age of 13. Access is obtained by completing the MyChart Access Authorization with Minor Proxy form. Pediatric Proxy Authorization Form and is automatically terminated on the child's 13th birthday.

Adult Proxy

Adult proxy access for MyChart allows you to securely communicate on behalf of an YNHHS/YMG patient who is 18 years of age or older. In order to obtain access, both the patient and proxy must complete and sign the MyChart Access Authorization with Adult Proxy. The proxy's access is terminated when the patient makes a written or online request to terminate access, an expiration date specified by the patient is reached, or the patient revokes access from within their MyChart account.

Download the appropriate form and fax to 203-688-0289 or email a scanned completed copy to MyChartSupport@ynhh.org
You will be notified by Medical Records regarding your proxy access in approximately 10 business days.

To access the Proxied chart

1. Login to your MyChart account from our secure web page with your MyChart Username and password

2. Click on "Family Access Settings"

3. Activate the patient by clicking on the patient's name
